ASDAH 2023 Annual Conference Evaluation

Thank you for attending the 2023 ASDAH Conference! We’d love to hear what you’re taking away from this conference through this evaluation form. Use this form if you are requesting a CDR or APA certificate of completion or a certificate of attendance for other credentialing organizations. Approval is still pending for CDR and APA continuing education credits for recordings. Please allow 5 business days to process CE certificates and certificates of attendance.

CDR Pre-Approved Continuing Education Credit Options:

  • Certificate of Completion for 8.5 CDR-approved live credits (must attend all for-credit sessions, listed below)
  • Certificate of Completion for 9.5 CDR-approved recorded credits (must attend all for-credit sessions, listed below)
 
APA Pre-Approved Continuing Education Credit Options:
  • Certificate of Completion for up to 8.5 APA-approved live credits (can attend any number of for-credit sessions)
  • Certificate of Completion for up to 9.5 APA-approved recorded credits (can attend any number of for-credit sessions)
 
Continuing Education Credit Options for Professions not Covered by CDR or APA:
 
  • Certificate of Attendance for up to 11.5 contact hours (this option supports submissions for any professions not covered by the CDR or APA)
